Adeola Fakolade, MD Family Medicine Adeola Fakolade, MD, is welcoming new patients at Ashtabula Family Health Center and will soon begin seeing patients at Orwell Family Health Center. Dr. Fakolade completed her residency at University of Pittsburgh Medical Center McKeesport. She recently completed a preventive medicine and public health fellowship at Case Western Reserve University/University Hospitals Cleveland Medical Center, where she was a chief fellow and focused on primary care in rural Ohio and strategies to combat diabetes. Dr. Fakolade has also worked with veterans dealing with hypertension and diabetes. She has a special interest in general primary care, preventive screenings, hypertension, diabetes mellitus, smoking cessation, weight management, basic joint injections, cryotherapy and female examinations. Carolyn Kwan, MD Family Medicine Carolyn Kwan, MD, sees patients at Ashtabula Family Health Center. She comes to ACMC from Summa Health System–Barberton Campus, where she completed her residency. Dr. Kwan earned her medical degree from Ross University School of Medicine. Dr. Kwan has served as a clinical instructor of family and community medicine at Northeast Ohio Medical University. Dr. Kwan is interested in a variety of treatment and procedure options for patients, including joint injections; cryotherapy of warts and moles; Nexplanon insertion and removal; toenail, mole and cyst removal; laceration repairs; and incision and drainage.
